FORMAT & INFO:   June 23, 2012 at Fiesta Island
Men’s: Open, Masters Division
Women’s: Open (must have 8 teams for a div.)

ROUND ROBIN play with up to four (4) teams each court progressing to: "A" Division Single Elimination and “B” Division Single Elimination Playoff. Prizes to the top 2 teams in each Division (must have 8 teams in a Division for a 2nd Place Award). "A" Division will get prize money. Other divisions will win nice prizes. Participant gift will be tshirts. Please see full application for full tourney details.

IMPORTANT!
Please be VERY CAREFUL to choose the appropriate "ADD TO CART" buttons here for the correct amount to charge (depending on division), or your team entry may be disqualified and you will not be pre-apped for any Participant commemorative tshirt gift.

Please fill this out as completely as possible. It's important to include your teammate's email address also, so we can include them on last minute tourney updates or emergency information.

TEAM ENTRY IS 75.00 for Men's Open 'A' .... and
$60 per team for everyone else.

Note: If you pay online, you will go to a secure Paypal website. (You must sign up for a Paypal account if you don't have one already - this may take 2-3 days to confirm.) Do it - it's not hard at all!

No online entries accepted after Thursday June 21st - we must organize tourney. Just be a 7:15am Walk-In.

Add $2.00 for every XXL t-shirt size needed (remember to change your amounts if it's more than one) = $2. each

Add $10.00 for any extra t-shirt you might want (just indicate your quantities). Don't forget to add $2 for any XXLs.

RELEASE & HOLD HARMLESS AGREEMENT

  I hereby release and promise to hold the OTLPA and the City of San Diego (COSD) harmless from any and all claims, actions, demands, rights, damages, costs, expenses or compensation for any injury to myself, my own personal property or any personal property in which I may have an interest, while I am participating in an OTLPA event or activity during the OTL season. I intend this release to be construed as broadly as possibly under California law, and if any part of it is held to be invalid or unenforceable, I agree that the remaining parts will remain effective.
  I am fully aware of the risks and safety hazards, which are inherent in the sport of over-the-line and related activities and I voluntarily assume them.
  I understand the OTLPA is a non-profit organization, staffed entirely by volunteers and operated for the benefit of its members and does not maintain liability insurance (or a reserve for liability coverage) for any of its events and/or activities.
  This Release and Hold Harmless Agreement shall be binding on my heirs, executive administration, successors, assigns and transferees and shall inure to the benefit of OTLPA or the COSD or its officers, directors, members, employees, agents, contractors, suppliers, attorneys, successors, assigns and any other individual and/or entity affiliated with the OTLPA of COSD.
  By entering this tournament, I agree to this Release automatically.
