Men’s: Open, Masters (35+), Connoisseur (50+),
Women’s: Open, Champagne (35+)
(must have 8 teams for a div.) CONNOISSEUR and CHAMPAGNE entries get a further discount - see special payment button for you below. Each Pre-App'd Participant will receive a Divisionals tourney hat.
ROUND ROBIN play with up to four (4) teams each court progressing to: "A" Division Single Elimination and “B” Division Single Elimination Playoff. Prizes to the top 2 teams in each Division (must have 10 teams in a Division for a 2nd Place Award). Participant gifts. Please see full application for full tourney details.

Please be VERY CAREFUL to choose the appropriate "ADD TO CART" buttons here for the correct amount to charge (depending on date we receive it), or your team entry may be disqualified and you will not be pre-apped for any Participant gifts or discounts. You should know if your teammates are paid-up OTLPA members by now. (How many years you been playing?)
EARLY TEAM ENTRY IS $78.00 !!
We give a $6 discount to all our members! These buttons are set up assuming you are members. Non-members, please add the $6 button each. Anyone can play!
Please fill this out as completely as possible. It's important to include your teammate's email address also, so we can include them on last minute tourney updates or emergency information.
Note: If you pay online, you will go to a secure Paypal account. (You must sign up for a Paypal account if you don't have one already - this may take 2-3 days to confirm.)
Late Entry $70.00 / team, if received AFTER Aug. 14th, (a small $4 cc transaction fee will be added automatically) = $74.00 (no Pre-App gift either - the stock will have already been ordered and imprinted - you will be past deadline)
CONNOISSEUR & CHAMPAGNE DIVISION = $55.00 / team (if all players are paid-up OTLPA members), and if received by Aug. 14th (a small $3 cc transaction fee will be added automatically) = $58.00
Add $6.00 for each non-OTLPA-member (remember to change your amounts if it's more than one) = $6. each
